Pledge Form

  • Distributed to every person
  • Please make sure they are legible
  • Social Security Number is required (unless cash/check contribution)
  • Donor Signature (for payroll deduction)
  • Pink copy to donor/contributor
  • White copy to Payroll Services Department (very important)
  • Yellow copy to LFCC Rep (Terra Donaldson)

Key Worker Envelope Instructions

  • Complete ALL the following information on the Key Worker Envelope:
    • Check Your Department or Agency Name for Accuracy
    • Insert Your Full Name
    • Telephone Number
    • Full Name and Address of Your Main Payroll Center
    • Number of Employees in Your Department or Command
    • Total number of Pledge Forms in the Envelope
    • Total Dollar Amount of Payroll Deduction Pledge Forms in the Envelope
    • Total Dollar amount of Cash and Checks in the Envelope
    • Total Dollar Amount of BOTH payroll deductions and cash/check contributions.

Submit the white copies to your PSD (Payroll Services Department) representative and have that representative sign the key worker envelope to confirm they have received the Pledge Forms. (This procedure must be completed prior to delivering yellow copies of the pledge forms to the CFC representative).
